Important information from district nurse Andrea Lewis

Here's what parents need to know about obtaining medication forms for their students and about a required immunization for seventh-graders and 12th-graders.



Students medication forms are available at Parents may also pick up copies of the forms from district nurse Andrea Lewis at the Secondary Campus Open House on Thursday, Aug. 18, 6 -8 p.m. The forms include student prescription medication forms, over-the-counter medication forms and forms related to specific health conditions.

This is a reminder that incoming 7th graders and 12th graders are required to be vaccinated against meningococcal (serogroups A,C,W and Y) disease. One dose of meningococcal vaccine is required for 7th graders and two doses are required for incoming 12th graders unless the initial dose was given on or after the 16th birthday.

If your child has not already received the vaccine, please contact your child's doctor to schedule an appointment. The Health Department provides the vaccine either free of charge or for a nominal fee for children without insurance or with Medicaid insurance.  You may reach the Health Department at (513) 956-7882 for additional information.
